Form 1 - Application for Miners Right (.pdf 140kb)
To prospect for minerals or peg out a mineral claim, you will need to be the holder of a current Miners Right. Fee required.

Form 2 - Application to Renew Miners Right (.pdf 150kb)
Use this form if you to renew your Miner's Right. Please note that a fee is required.

Form 5 - Application for Registration of a Mineral Claim (.pdf 220kb)
If you have pegged out a mineral claim, you must make an application for registration of the mineral claim within 30 days from the date of pegging. Please note there is a fee required.

Form 10 - Application for Mining Lease (.pdf 230kb)
If it is found that your mineral claim has an economic deposit of minerals, use this form to apply for a mining lease, which must be obtained before production can commence. Please note that a fee is required.

Form 11 - Statement of Proposed Mining Operations and Measures to Remedy Damage (.pdf 210kb)
This form must be lodged together with Form 10. You must state the nature of your proposed mining operations and describe what measures you will be taking to remedy damage to the land.

Form 12 - Application for Retention Lease (.pdf 280kb)
If, for economic or other reasons, it is not possible to work a deposit immediately you may apply for a retention lease. Fee required.

Form 13 - Instrument of Transfer - Lease (.pdf 155kb)
Ministerial consent is required for the transfer of a lease to another party. Both parties must fill out this form. Fee required.

Form 14 - Surrender (or Partial Surrender) of Lease (.pdf 140kb)
Ministerial consent is required to the surrender or partial surrender of a lease. This form must be filled out and in the case of a partial surrender must be lodged together with a plan which delineates the area that is no longer required.

Form 15 - Application for Renewal of Mining Lease (.pdf 155kb)
An application for renewal of a mining lease must be lodged no more than 6 months, and not less than 3 months before the date of its expiry. Fee required

Form 16 - Application for Renewal of Retention Lease (.pdf 160kb)
An application for renewal of a mining lease must be lodged no more than 6 months, and not less than 3 months before the date of its expiry. Fee required.

Form 17 - Application for Miscellaneous Purposes Licence (.pdf 170kb)
Land that will be used as a treatment/operating plant, provide amenities for persons engaged in mining operations, for the disposal of overburden, for drainage from a mine or any other purpose ancillary to the conduct of mining operations may be covered by a Miscellaneous Purposes Licence. Fee required

Form 18 - Application for Renewal of Miscellaneous Purposes Licence (.pdf 125kb)
An application for renewal of a mining lease must be lodged no more than 6 months, and not less than 3 months before the date of its expiry. Fee required

Form 19 - Instrument of Transfer - Exploration or Miscellaneous Purposes Licence (.pdf 165kb)
Ministerial consent is required for the transfer of a licence to another party. Both parties must fill out this form. Fee required.

Form 21 - Notice of Entry (.pdf 270kb)
A miner must give a landowner at least 21 days notice of his/her intention to enter land to prospect or mine. This form must be filled out and forwarded to the landowner. You must keep a copy of the completed form for your records.

Form 22 - Notice of Use of Declared Equipment (.pdf 165kb)
A miner must give a landowner at least 21 days notice of his/her intention to use declared equipment. This form must be filled out and forwarded to the landowner. You must keep a copy of the completed form for your records.

Form 23 - Waiver of Exemption (.pdf 200kb)
Certain land is exempt from prospecting and mining. A miner cannot mine or prospect on exempt land until the owner of the land agrees and signs a waiver of exemption. This form must be filled out and lodged with the Mining Registrar.

Form 24 - Caveat Against a Mining Tenement (.pdf 155kb)
A person claiming an interest in a mining tenement may lodge with a mining registrar a caveat forbidding the registration of any transfer or other instrument affecting the mining tenement or interest. Fee required

Form 25 - Caveat By Consent (.pdf 145kb)
This form is to be used if you are transferring a tenement and you wish to bar the transfer or assignment of the tenement during the period pending the completion of the transfer application. Fee required

Form 27 - Notice Initiating Negotiations with Native Title Parties Section 63M (.pdf 170kb)
Use this form if you are in a position to initiate your negotiations with native title parties as required by section 63M of the Mining Act, 1971.

Form 29 - Application for Mineral Exploration Licence (.pdf 110kb)
Large-scale exploration is conducted on an Exploration Licence. This form must be lodged together with the relevant fee.
